Northern Beaches Neurology is committed to protecting the privacy of the personal information and sensitive information which it collects and holds.

Northern Beaches Neurology must comply with the Australian Privacy Act 1988(Clth), and other privacy laws which govern the way in which organisations hold, use and disclose personal information.

The purpose of this policy is to explain:

  2. How Northern Beaches Neurology collects and holds personal information.

    Northern Beaches Neurology collects information from you to facilitate the provision of health care services to you by members of the Northern Beaches Neurology Team. This includes collection personal information such as your name, address, your health history, family history, past and current treatments, lifestyle factors and any other information which is necessary to assist the health care team in providing appropriate care.
  2. Contractors, including medical:
    Northern Beaches Neurology collects information from you which includes you name, address, professional experience, qualifications and past employers and any other information which may be necessary to appropriately conduct, manage and oversee the Practice.
  3. Jobapplicants:
    Northern Beaches Neurology collects information from you which is necessary to assess and engage job applicants. This includes collecting personal information such as your name, address, professional experience, qualifications, references and past employers, and any other information which is necessary to process your job application.

2.2. How we collect personal information

Northern Beaches Neurology will usually collect your personal information directly from you, however, sometimes we may need to collect information about you from third parties, such as:

  1. Anotherhealthserviceprovider
  2. Past employers and referees; or
  3. Related entities such as the Hospital Clinic

We will only collect information from third parties where:

  2. Such collection is necessary to enable us to facilitate the provision of appropriate health care services by associated care providers

2.3. How Northern Beaches Neurology uses your personal information

Northern Beaches Neurology only uses your personal information for the purpose for which it was collected, unless:

  1. There is another purpose and that secondary purpose is directly related to the primary purpose, and you would reasonably expect, or Northern Beaches Neurology has informed you, that your information will be used for that secondary purpose;

Northern Beaches Neurology will confine its disclosure of your personal information to the primary purpose for which that information has been collected, or for a related secondary purpose. This includes when disclosure is necessary to facilitate the provision of health care services to you by a member of the Northern Beaches Neurology Team. Northern Beaches Neurology strives to ensure the security, integrity and privacy of personal information, and will take reasonable steps to. Northern Beaches Neurology reviews and updates (where necessary) its security measures in light of current technologies.

4.4. Data Breach

In the event of a data breach, a response plan is in place to assess and contain breaches and to reduce the impact of a breach (data breach response framework available upon request). The plan includes notifying individuals affected and the Office of the Australian Information Commissioner if required.

4.5. Accessing and amending your personal information

You have a right to access your personal information which Northern Beaches Neurology holds about you. If you make a request to access your personal information, we will ask you to verify your identify and specify the information your required.

You can also require an amendment to any of your personal information if you consider that it contains inaccurate information.

While Northern Beaches Neurology aims to meet all requests for access to personal information, in a small number of cases and where permitted to do so by law, Northern Beaches Neurology may not give access or may do so only under conditions.

Subject to applicable laws, Northern Beaches Neurology may destroy records containing personal information when the record is no longer required.

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals
